MLB.tv on Roku entered a public beta on Monday. I think you still need to contact Roku via their forums to get the updated channel, but it is working for many users. Its a beta still though, so some issues may crop up.

If we could only get the NFL to lift their limit of computer streaming of overseas or in 2009 Manhattan and make it available to more US viewers and platforms. DirecTV's latest contract requires them to offer an online service by 2012. My guess is that their Manhattan service was the testbed for that in 2009.
